Sign Notary Public Commission Application Oath
of Office. Please sign your name, as you want your name to appear on your
commission.

Have someone you have known for at least one
year and who is not related to you, complete the Affidavit of Character.

Sign your name by the “X” on the Bond of
Notary Public as you signed your Oath of Office.
-
Mail the completed forms and a check in the
amount of $118.00 (plus tax) made payable to:
